Distal Forearm Fractures

Lecture´s Description

Prime emphasis of this sqadia.com medical video lecture is  the elaboration of fractures of distal radius. Dr. Hammad Haider talks about Colles’ fracture initially. Then focus is shifted towards undisplaced and displaced fractures. Next comes volar displaced fracture and fractured radial styloid. Moreover, comminuted intra-articular fractures along with distal forearm fractures are also reported.

Colles’ Fracture

Fractures of the distal radius in adults include Colles’, Smith’s or Barton’s fracture. Colles’ Fracture is also known as low-energy dorsally displaced fractures and have high incidence related to onset of postmenopausal osteoporosis. Mechanism of injury and pathological anatomy is pursued later on. Orthopaedic surgeon also tells that what imaging results demonstrates for better understanding.

Undisplaced and Displaced Fractures

Dr. Hammad Haider notifies that in undisplaced fractures, dorsal splint in placed for a day or two until swelling resolves. After that displaced fractures comes under consideration. Fracture line exits transversely in central part of distal radius joint. Subsequently, information about volar locking plates is given which allow stable fixation and early mobilization but are quite expensive. Following this, K-wires are also discussed which are cheaper.

Volar Displaced Fracture and Fractured Radial Styloid

Smith’s fracture also called as volar displaced fracture is primarily discussed in this section which is caused by fall on back of hand. Wrist injury, garden-spade’ deformity are the clinical features. Afterwards, fractured radial styloid is reported which is associated with carpal ligament injury. Moreover, Dr. Hammad Haider also brings into consideration the treatment modality.

Comminuted Intra-articular Fractures

Intra-articular fractures in adults usually involve either the volar or dorsal lip at the end of the bone. It is associated with high frequency of post-traumatic arthritis. Early complication includes circulatory problems, complex regional pain syndrome (CRPS), ulnar corner pain and instability. Some of the late complications encompass malunion, tendon rupture, and secondary osteoarthritis.

Distal Forearm Fractures

Distal radius and ulna are common sites of childhood fractures. This is followed by comprehensive elaboration of mechanism of injury. Clinical features comprise dinner-fork’ deformity, painful and swollen wrist. Furthermore, undisplaced or buckle/torus fractures are highlighted. Information about greenstick fractures along with complete fractures is also conveyed. Some early and late complications are also reported.
